1. Order your Midwest Stylus handpieces through authorized DENTSPLY Distributor.
2. Call DENTSPLY customer service at 1-800-800-7202, press menu option 2, and ask for a Product Repair (PR) number. DENTSPLY will
place the order for you.
3. Autoclave the handpiece that you are using as the trade-in, write your name, address, and phone number on the sealed sterilized bag.
Package the trade-in handpiece in the box that contained your new Stylus, include a copy of the Distributor invoice showing the
purchase of Stylus, write the PR number on the invoice, and apply the shipping label.
4.Write the PR number on the outside of the shipping box.
5. Ship the trade-in handpiece to Midwest Customer Service, 901 West Oakton Street, Des Plaines, IL 60018.
